We describe the case of a 'male' patient who presented with a mass in the right side of the pelvis, gynecomastia and absence of the right testis in the scrotum. On US and CT the mass was diagnosed as a tumour in relation to the undescended testis. Laparotomy and histopathology revealed the presence of a female genital tract with, a tubo-ovarian mass of tuberculous etiology.
