Rheumatoid arthritis, amyloidosis, and chylous effusions

A patient with rheumatoid arthritis and secondary amyloidosis is described. The initial clinical expression of the amyloidosis was bilateral chylous pleural effusions due to intrinsic amyloid obstruction of the lymphatics. This is an unusual initial manifestation of secondary amyloidosis.
