It appears below. K. Marie Douglass, BA Ann Polcari, MS Nadine Najjar, BS Joshua Kronenfeld, MD Amar R. Deshpande, MD 1) Lotus Wellness Center, LLC is a wholly owned subsidiary of Sundari Foundation, Inc. and operates the Lotus Wellness Center. Lotus Wellness Center, LLC is the legal owner of the Lotus Wellness Center. 2) The phrase “student-run clinic” refers solely to the volunteer activities of the DOCS in Lotus Wellness Center; descriptions included in the paper, including the logic model, do not reflect the actual legal or organizational structure or operations of Lotus Wellness Center. The students do not serve on the managing board of the Sundari Foundation, Inc. or Lotus Wellness Center, LLC. 3) Lotus House shelters 250 women and children nightly, not annually. 4) Lotus House provides not just transgender care but services for many other populations, including LGBTQ individuals; some, but not all, were eligible and elected to obtain primary care via Lotus Wellness Center.
