PONV remains a problem for many patients. Key risk factors for PONV are: female gender, opioid use, no smoking and a history of PONV/motion sickness. 5HT3 antagonists are well suited for first line agents due to their high efficacy and good side-effect profile. Combination therapy significantly improves efficacy. Generally, agents that fail for prophylaxis should not be repeated for rescue.
