Abstract

Cough and difficult expectoration of viscid airway secretions are common features in bronchial asthma. This suggests abnormalities in the production and clearance, and hence accumulation, of mucus in the airways. In the article, structural and fuctional characteristics determining physiologic mucociliary clearance are taken into consideration. The dependance of the abnormalities in mucus transport from the alterations of ciliary beating, rheologic mucus properties, abnormal periciliary water flux and muco-ciliary interaction is then examined. In the article such abnormal features, present in asthma, are "dissected" and their cause-effect relationships assessed. During the asthmatic attack we initially assist to a temporary process of leukotriene-dependent cilio-stimulation. Rapidly following this phenomenon, during the asthmatic attack as well as in the most severe clinical conditions of asthma, we assist to mucus hypersecretion accompanied by increased transepithelial water transport, both phenomena concurring to the impairment which is observed in such clinical conditions. Accumulation of secretions (furthermore rheologically alterated) in the airways is consequent. Often, if the conditions determining these phenomena result repetitive, the airways easily undergo superinfection, and chronicization with consequent signs and symptoms of bronchoconstriction, either inercritically.
